Newsletter Vol. 2 Aug 2020

We are the proposed Oxford and Area Ontario Health Team (OHT). The
Ontario government is establishing Ontario Health Teams across the
province to build a connected health care system centred around patients,
families and caregivers.

OHTs bring together primary care providers, hospitals, community and home
care providers to collaborate and deliver integrated care. This connected
team will make it easier for patients to navigate the health care system, and
receive the right care in the right setting.

While the novel coronavirus (COVID-19) pandemic has overshadowed some
of this work, it has also underscored the need for OHTs. The partnerships
formed through OHTs will enhance our ability to rapidly and effectively
respond to COVID-19.

Newsletter Volume #1

In February 2019, the provincial government introduced The People’s Health Care Act, 2019. This act supports the establishment of local Ontario Health Teams to connect and integrate the full continuum of care; health care providers and services for patients and families. Amid this province-wide integration initiative, Oxford County and area will be taking steps to create an Ontario Health Team.

The purpose of this newsletter is to inform and engage Oxford and area providers we embark on the journey to create the Oxford Ontario Health Team.

Proposed Oxford Region Ontario Health Team

In response to the open call by the Ontario Government for readiness self-assessments for Ontario Health Teams (OHT), a team of more than 20 health and community care partners in Oxford have taken the first step toward designation as an OHT by submitting the required Self-Assessment document. By leveraging existing relationships, this team is dedicated to the modernization of healthcare for the residents of Oxford County. “We are confident in our ability to deliver on the vision of Ontario Health.” said Randy Peltz, Executive Director of the Oxford County Community Health Centre.
